Notification of donor care is a requirement under the National Registry of Diseases Act. You can submit a notification either by post or electronically.

Health Professional Portal Login

A portal for health professionals to notify us about reportable diseases and conditions.

●  Nurses' Login
For Nurses with HPP login issue, please login to the Singapore Nursing Board Professional Registration System (PRS) to re-activate your user account if you have not login for more than 90 days.

Paper notification

To notify the registry using a hardcopy, please download the relevant form:
●    New Case Registration Form for Kidney Donor
●    Yearly Follow-up Form for Kidney Donor
●    New Case Registration Form for Liver Donor
●    Follow-up Form for Liver Donor
 
Please send it via registered post to:
